Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Administer, evaluate, and interpret tests to determine functional goals and outcomes, then provide written evaluations and plans of care defining needs, progress, and expected results. Deliver physical therapy services complying with physician orders while maintaining plans through timely documentation, goal modification, and discharge care planning. Collaborate with physicians, nurses, and caretakers on home exercise plans and coordinate services across the healthcare team. Complete OASIS assessments where permitted, supervise personnel like PTAs and HHAs, and ensure documentation meets payer standards to prevent denials. Maintain effective communication with staff and families while adhering to all licensure regulations and patient care standards.
