Salaried Physical Therapist Full Time 15K Sign On Bonus

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels for home health patients. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to set functional goals and outcomes. Provide written evaluations and plans of care, documenting client performance and modifying treatment as needed. Collaborate with physicians, nurses, and caretakers for discharge planning and education on home exercise programs. Supervise personnel including PTAs and HHAs where applicable. Complete OASIS assessments and ensure documentation meets payer standards to prevent denials. Maintain effective communication with the healthcare team and family. Requires a Physical Therapy degree, current state license, and CPR certification.
