Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy interventions and treatment directed by the physician within the home health plan of care. Provide written evaluations, modify goals and treatment as needed, and determine discharge care plans while documenting client performance to prevent technical or clinical denials. Collaborate with interdisciplinary team members, physicians, and caretakers to optimize patient well-being and educate families on home exercise plans. Supervise personnel including PTAs and HHAs where applicable. Complete OASIS assessments where allowed by state regulations.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Valid Drivers License
- Valid Automobile Insurance
- Full-time visits needed in South/Southeastern part of branch territory - Onamia, Isle, Wahkon, Little Falls, Pierz, Fort Ripley, and Hillman, MN
