Assess, teach, and deliver physical therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations and modify plans of care based on client performance, including discharge planning and educating caretakers on home exercise programs. Collaborate with interdisciplinary team members, physicians, and nurses to optimize patient well-being, supervise personnel as applicable, and complete OASIS assessments. Ensure all documentation complies with policies and payer standards to prevent denials. Full-time visits required in Garrison, Onamia, Wahkon, Isle, Aitkin, Deerwood, and Crosby, MN.
