Assess, teach, and deliver therapy interventions directed by the Physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans as needed, and coordinate services with physicians, nurses, and families. Supervise personnel including PTAs and HHAs, complete OASIS assessments, and ensure documentation meets payer standards to avoid denials. Full-time visits required in South/Southeastern branch territory of Minnesota with a $25,000 sign-on bonus and comprehensive benefits including medical, dental, vision, and 401(k) match.
