Assess, teach, and deliver physical therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations and modify plans of care, including discharge planning and educating caretakers on home exercise programs. Collaborate with interdisciplinary team members, physicians, and nurses to optimize patient well-being, supervise personnel as applicable, and complete OASIS assessments. Document client performance effectively to ensure compliance with physician orders and payer standards. Full-time visits required in Garrison, Onamia, Wahkon, Isle, Aitkin, Deerwood, and Crosby, MN.
