Assess, teach, and deliver physical therapy interventions directed by the physician to optimize patient well-being. Perform initial and ongoing assessments of mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans, and manage discharge care including educating families on home exercise plans. Collaborate with interdisciplinary teams, physicians, and caregivers; supervise PTAs and HHAs as applicable. Complete OASIS assessments and ensure documentation adheres to policies to prevent clinical denials. Maintain effective communication across staff and family members. Full-time visits required in Garrison, Onamia, Wahkon, Isle, Aitkin, Deerwood, and Crosby, MN.
