Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function. Administer, evaluate, and interpret tests to determine functional goals and outcomes, then provide a written evaluation and plan of care defining needs, goals, progress, and expected outcomes. Deliver occupational therapy services to comply with prescribed physician orders while maintaining plans through timely documentation and modifying goals or treatment as needed. Collaborate with the health care team, physicians, and caretakers for discharge planning and education on home exercise plans. Document according to policy and procedure to ensure no technical or clinical denials, and supervise other personnel where applicable. Daytime visits Monday through Friday in Winona, Wabasha, Buffalo, and Trempealeau.
