Job Summary
Occupational Therapist (OT) needed for home health in Cedar Rapids, IA, providing direct therapy services, assessments, and care planning across Linn and Johnson County communities. Deliver therapy interventions, develop and update care plans, perform OASIS where allowed, discharge planning, and collaborate with an interdisciplinary team. Maintain thorough documentation, communicate with patients, caregivers, and healthcare staff, and supervise OTA/HHA personnel as applicable. Includes benefits package and 401(k).
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
