Job Summary
Occupational Therapist (OT) for full-time, home-health services in the Lake City area. Responsibilities include performing initial and ongoing assessments of mobility, coordination, strength and current level of function; delivering OT services per physician orders; developing and maintaining a written evaluation and plan of care with goals and expected outcomes; coordinating services with the healthcare team; completing OASIS where allowed; supervising staff (OTA, HHA) as applicable; discharge planning and home-exercise education; and thorough documentation to support outcomes and reimbursements. Requires current OT license in WI/MN, CPR, and valid Driver's License; preferred experience in home health and OASIS documentation; strong collaboration and communication with patients, families, and care teams.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program.
- Current, unrestricted state license as an Occupational Therapist in the state of practice (WI and MN)
- Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
- Valid CPR and Drivers License
