Ryan White Program Nurse (LPN)

Coordinate primary care for Ryan White HIV/AIDS patients by completing initial histories, conducting confidential HIV testing and counseling, and performing phlebotomy. Develop individualized service plans, process positive referrals including intake and lab orders, and triage patient phone calls. Manage medication adherence, educate patients and families on disease prevention and coping, and assist with obtaining HIV medication coverage. Chart treatment plan changes, report positive tests to the Health Department, and support Quality Improvement protocols. Participate in internal and external program meetings while maintaining strict confidentiality. Requires current LPN or RN license with two years clinical experience, including phlebotomy.
