Job Summary
Manage an assigned caseload and maintain accurate, up-to-date case records for youth placed by the DOE and DSS, ensuring compliance with OCFS, DSS, ACS, CSE, and agency regulations. Complete required documentation including progress notes, FASPs, permanency reports, incident reports, and court-related reports while coordinating family engagement activities such as home assessments and sibling visits. Attend court hearings, treatment team meetings, FTCs, and multidisciplinary meetings, serving as a third-party reviewer for Service Plan Reviews and Family Treatment Conferences. Monitor case-related documents, evaluations, and entitlements, and facilitate referrals to community resources to support discharge planning. Respond to incidents including Justice Center matters and AWOL events, and attend required trainings to support program operations. This role operates within a 24/7 RTC serving youth ages 12-21 across multiple community-based residential sites with a 9am-5pm or 10am-6pm schedule, requiring regular travel to assigned locations.
Required Qualifications
- Bachelor's degree in Human Services, Social Work, Psychology, or a related field
- 1-3 years of experience working with adolescents in residential, child welfare, behavioral health, or related human services settings
- Valid driver's license with access to reliable transportation
- Strong verbal, written, and interpersonal communication skills
- Ability to manage multiple priorities, maintain organization, and work independently with minimal supervision
- Demonstrated empathy, flexibility, professionalism, and commitment to providing high-quality services to youth and families
- Authorized to work in the U.S.
- Ability to work in-person in New York City (NYC)
- Ability to travel to other Rising Ground Sites (if required)
Desired Qualifications
- Bachelor's degree in Human Services, Social Work, Psychology, or a related field preferred
- Bilingual (Spanish) preferred but not required
