Job Summary
Conduct comprehensive head-to-toe assessments and deliver services such as wound care, IV therapy, chest tube management, and diabetes management for clients 90–120 days following hospital discharge. Coordinate care plans in collaboration with hospital partners and Home Care Specialists, while performing ongoing reassessments to evaluate intervention effectiveness. Maintain documentation standards, inform supervisors of client changes, and participate in team meetings and a standby/on-call rotation. Work day shift from 09:00 am to 1:00 pm with bi-weekly weekend rotation.
Required Qualifications
- Registered and in good standing with the College of Nurses of Ontario (CNO)
- RPN diploma from a recognized college
- Valid First Aid or CPR Certification
- Valid Ontario driver's license (G License)
- A reliable vehicle for traveling
- Demonstrate effective communication (written and verbal) and documentation skills
- Demonstrate reliable attendance, punctuality, and availability for the role
- Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care
- Membership of your Professional Association preferred (WeRPN)
- Acute Care and Medical-Surgical nursing experience is an asset
- Knowledge of community nursing is an asset
