Conduct physical therapy evaluations and assessments, then develop and implement individualized treatment plans for patients in their homes. Provide skilled physical therapy interventions and educate patients and caregivers on treatment plans and home exercise programs. Complete accurate and timely documentation while communicating effectively with physicians, case managers, and interdisciplinary team members. Travel throughout assigned Los Angeles territories to evaluate and treat patients. Create your own schedule and accept or decline patient referrals based on availability.
