Job Summary
Assess participants' needs and plan nursing care upon initial and routine re-evaluation assessments. Collaborate with participants, families, and the multidisciplinary team to develop individualized plans of care that maximize functional capacity and encourage autonomy. Administer medications and treatments as ordered, monitor responses, and instruct families on safe administration. Teach participants and caregivers using adult learning principles while supervising home health aide services and documenting monthly outcomes. Lead licensed and non-professional staff in delivering coordinated care, evaluate progress toward care plan objectives, and revise plans based on changes in physical or psychosocial status. Maintain accurate records of functional status, care revisions, and PACE data collection. Provide rotating after-hours on-call medical assistance via phone triage or home visits. Requires current Michigan licensure, BSN preferred, and one year of experience with frail or elderly populations.
Required Qualifications
- Registered Nurse
- current Michigan licensure
- Must meet a standardized set of competencies (approved by CMS) before working independently
- Must have one (1) year of experience with a frail or elderly population
- current State of Michigan driver's license
- acceptable driving record
- Must be medically cleared for communicable diseases
- all immunizations up-to-date
- Must have reliable transportation available on a daily basis
- Must be able to lift forty (40) pounds or more
