RNAC\LPNAC Registered Nurse or Licensed Practical Nurse

RNAC/LPNAC role delivering resident care in a long-term care/rehabilitation center. Responsibilities include conducting and coordinating the resident assessment (MDS) per rules and guidelines, maintaining and updating policies and procedures governing MDS and care plans, developing and maintaining a quality assurance program for resident assessment/care plans, assisting with discharge portions of care plans, participating in facility surveys by government agencies, developing preliminary and comprehensive nursing assessments, auditing documentation for standard of practice, auditing MDS and care plans for accuracy/completeness, auditing the CMI for accuracy, and notifying the DON and administrator of problems promptly. Requires current RN or LPN licensure and prior healthcare experience; long-term care or rehabilitation experience preferred.
