Registered Nurse Assessment Coordinator

Conduct and coordinate the development and accurate completion of resident assessment and care plan processes in compliance with federal, state, and local guidelines. Schedule OBRA and Medicare/managed care assessments, manage entry/re-entry records, and oversee care plan reviews. Inform residents and staff of coverage status through appropriate documentation while working with Community Health Choice case managers. Maintain in-depth knowledge of Pennsylvania case mix, Medicare PDPM payment systems, and quality measures. This role supports regulatory compliance and survey readiness within Meadow View Nursing Center's collaborative team in Berlin, PA.
