Provide skilled nursing care to patients in their homes including wound care, IV maintenance, and patient/family education. Monitor patient conditions and document care provided while administering medications and treatments as prescribed. Educate patients and families on care plans and health management, and communicate effectively with physicians and care team members. Maintain accurate and timely patient records. On call weekend every 3-4 weeks. Valid RN or LPN license in [State], current CPR certification, and reliable transportation required. Home health experience preferred.
