Registered Nurse (RN) needed for Summit Home Care and Hospice to join our Home Health team. Responsibilities include planning, organizing and directing home care services; conducting initial assessments and ongoing re-evaluations; developing and updating care plans; administering medications; counseling patients and families; coordinating patient care as needed; communicating with physicians and care teams; and participating in on-call duties. Requires strong patient-care skills, home health experience, ability to work across community settings, and proficiency in care coordination, documentation, and family education. Travel is required with mileage reimbursement; position entails on-the-road patient visits and occasional remote work from the local office. Qualifications include an associate or bachelor’s degree in nursing, active nursing license, valid CPR, recent TB test, and a valid driver’s license; prior home health or acute care experience preferred.
