Job Summary
Provide nursing and supportive care to terminally ill patients in home hospice settings. Responsible for initial assessment of patient/family needs, developing and implementing the care plan with the Interdisciplinary Group, delivering direct nursing care, teaching family members or caregivers, coordinating equipment for home use, and supporting patient/family through end-of-life care. Requires Maryland RN license with 1-2 years of nursing experience (home health/hospice preferred); weekend 8am-8pm schedule; strong clinical skills, time management, and interpersonal abilities; proficiency with EMR and Blackberry; adherence to GBMC values and a patient/family-centered approach.
Required Qualifications
- Current Maryland State license as a Registered Nurse
- Education: Diploma, ADN and/or BSN from an accredited institution
- Experience: One to two years of nursing (Home Health or Hospice preferred)
