Job Summary
Coordinate complex wound care, PICC line management, and TPN infusions while performing skilled nursing assessments and patient education across Champaign and surrounding counties. Manage the full nursing process from admission documentation and OASIS assessments to discharge planning, ensuring timely physician order transmission and accurate record-keeping. Supervise Home Health Aides, conduct interdisciplinary case conferencing, and monitor patient conditions to promote independence. Requires IL RN license, BLS certification, and one year of nursing experience.
