Job Summary
Conduct home health visits to administer services, perform comprehensive assessments, and manage patient treatment plans. Administer IVs, change dressings, clean wounds, and document findings by uploading to the Synergy Software portal. Coordinate care with the Clinical Team Manager, physician, and other team members, initiating and revising plans of care as needed. Supervise Home Health Aides and handle re-certifications, transfers, and discharges in compliance with regulatory requirements. Requires current CA RN license, BLS certification, and IV/Phlebotomy credentials. Work 7 days a week with flexible scheduling for at least two-three days weekly, one weekend, and two holidays annually.
Required Qualifications
- Visiting RN for Home Health: 1 year
- Nursing: 1 year
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
- ability to work at least two-three days each week
- ability to work one weekend a month
- ability to work two holidays per year (one winter and one summer)
- nursing documentation/charting skills
