RN Visit Nurse- Osseo

Registered Nurse visiting patients in their homes (Osseo, WI) to provide and direct nursing care as prescribed by the physician and in compliance with laws and agency policies. Coordinate total plan of care with the care team from admission to discharge; perform clinical nursing assessments to meet payer and program requirements; determine patient eligibility and medical necessity for services; develop and revise individualized care plans with other providers; ensure plan of care frequency and duration meets patient needs and obtain physician approval for revisions; role emphasizes high-quality clinical services, infection-control standards, and collaboration with clinicians, families, and community resources. Requires RN licensure, a valid driver's license, access to reliable transportation, and CPR certification. Preferred: 1+ years RN experience, ability to work independently, strong communication, writing, and organizational skills. Pay ranges from $32.25 to $48.40 per hour for full-time!
