Job Summary
Oversee daily clinical operations of an assigned nursing unit, supervising RNs, LVNs, and CNAs to ensure accurate physician order implementation and timely resident assessments. Monitor resident conditions, respond to clinical changes, and lead interdisciplinary collaboration with physicians, therapists, and social services. Promote excellence in wound care, infection prevention, and fall prevention while auditing documentation for CMS and CDPH compliance. Participate in Quality Assurance and Performance Improvement initiatives, assist with admissions and transitions, and mentor staff to maintain survey readiness. Requires current California RN license, one year of skilled nursing experience, and BLS/CPR certification. Located in Los Altos, CA at a 152-bed skilled nursing and rehabilitation center dedicated to post-acute and long-term care recovery.
Required Qualifications
- Current, unrestricted California Registered Nurse (RN) license
- Current BLS/CPR certification
Desired Qualifications
- Minimum one (1) year of RN experience in a Skilled Nursing Facility or Post-Acute setting preferred
- Previous Unit Manager, Charge Nurse, or nursing leadership experience preferred
- Strong knowledge of skilled nursing regulations, MDS processes, infection prevention, and resident assessments
- Experience with PointClickCare preferred
- Excellent leadership, communication, organization, and critical thinking skills
