Job Summary
Develop individual care plans and conduct admissions, discharges, and other assessments for residents requiring long-term therapy and skilled nursing care. Manage acute health changes, physician orders, and interdisciplinary collaboration while ensuring compliance with federal, state, and company health and safety regulations. Audit documentation for quality control and serve as a liaison between families, providers, and the team. Provide ongoing training, orientation, and performance evaluations for staff, manage staffing schedules and assignments, and lead the unit in a long-term care setting. Requires Connecticut RN licenship, leadership experience, and background check clearance. Located in Naugatuck, CT.
Required Qualifications
- Prior Long Term Care Experience
- Licensed to work within the State of Connecticut as a RN Registered Nurse
- Able to pass background check, drug screen, professional references check & physical evaluation
- High level understanding of leadership, customer service and clinical competency
- Leadership experience in a long-term care setting
