Job Summary
Under the direction of the Supervisor, conduct patient care needs assessment via inbound and outbound telephone practice using active listening and clinical judgment. Manage immediate and ongoing patient needs through acute and preventative interventions, including phone triage, patient education, continuous Glucose Monitor placement, testing, insurance pre-authorization, and referrals. Clean and stock exam rooms as part of daily operations. Requires current Illinois RN licensure, CPR certification, and ability to lift 40 lbs. Schedule includes two 10-hour shifts Monday through Friday from 7:00 AM to 5:00 PM, with some travel to other locations and flexible hours.
Required Qualifications
- Current Illinois RN licensure
- Current CPR Certification issued by American Heart Association (AHA)
- Able to effectively communicate in English, both written & verbal
- Able to lift up to 40 lbs. and carry/push up to 20 lbs.
- Self-starter, able to handle periods of high stress
- Travel Required
- Flexible hours to meet operational needs
- Monday - Friday 7:00 AM -5:00 PM
- Two, 10-Hour shifts
- 20 hours / week
