Job Summary
Triage Clinical Coordinator for after-hours care delivers tiered nursing support via telehealth and phone, guiding patients, families, and community partners. The role coordinates information across teams, monitors workflow, ensures timely documentation and reporting, and provides clinical guidance to maximize comfort and quality of life for hospice patients. Requires an active Florida RN license, CPR, and at least one year of acute-care experience; CHPN encouraged within three years; BSN/MSN preferred. The position involves after-hours coverage (Thu-Sun, 5p-1a) and requires living near Lakewood Ranch, FL, with in-person collaboration and direct patient care as part of Empath Health’s Full Life Care for All initiative.
Required Qualifications
- Graduate of approved nursing program
- Current unrestricted Florida RN license
- Valid CPR certification
- Minimum one year experience in an acute-care Hospital environment providing direct patient care
- CHPN certification strongly encouraged within three years of employment
- BSN or MSN preferred
