RN - Transplant Coordinator

Job Summary

Conduct nursing care for pediatric and young adult patients before and after kidney transplantation, providing 24/7 coverage across outpatient and inpatient settings. Coordinate the full transplant continuum from pre-transplant evaluation through post-transplant follow-up, including interpreting eligibility criteria and assessing organ function. Collaborate with multidisciplinary teams to develop care protocols, participate in rounds and conferences, and lead education for patients, families, and nursing staff. Complete documentation in compliance with OPTN and CMS regulations while monitoring quality improvement needs. Provide on-call coordination for donor availability and discharge planning, including referrals to home health and infusion therapy.

Required Qualifications

  • Current Virginia state license as a Registered Nurse (RN) or Registered Nurse holding a valid Compact State license
  • Certified Clinical Transplant Coordinator (CCTC) required within three (3) years of hire
  • Bachelor of Science in Nursing (BSN) or relevant area from an accredited college or university or equivalent combination of experience and education
  • Technical and professional nursing knowledge, skills, and abilities acquired through a minimum of two (2) years of progressive acute care experience in solid organ transplant, critical care, or end-stage kidney disease
  • Attention to detail and effective communication and documentation with numerous physicians within and outside the health system, organ procurement organizations (OPOs), living donor recovery hospitals, histocompatibility laboratories, operating room (OR), pediatric intensive care unit (PICU), inpatient nursing units, outpatient clinic staff, infusion center staff, guardians/parents of patients, patients and family
  • Strong proficiency and technical aptitude in MS Office products, including Excel, PowerPoint, Outlook and Word
  • Models professional nursing through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, and problem-solving skills

Desired Qualifications

  • Certified Pediatric Nurse (CPN)
  • Two (2) years of solid organ transplant related nursing experience
  • Two (2) years of pediatric nursing acute care experience
  • Master of Science in Nursing (MSN) or relevant area
