RN, Transfer Center Coordinator (Part-Time, Night Shift)

Job Summary
RN Transfer Center Coordinator coordinates patient transfers and admissions by following pre-established criteria, performs telephonic patient triage and discharge follow-up, collaborates with ER, PACU, and bed management to optimize census and acuity, ensures orders are transmitted to nursing units, and documents patient interactions in EMR. Responsible for coordinating physician consultation and transport (ground/air), creating reports from patient data, and performing other duties as assigned in a part-time, night-shift role.
Required Qualifications
- Bachelor's Degree in Nursing (BSN): Required or Preferred?
- Registered Nurse (RN) licensure in the state of practice: Required
- Experience as a lead or charge nurse or with placement or care of critically ill patients: Required
- Experience working with electronic medical records (EMR): Required
