Job Summary
RN per diem providing skilled nursing care in patients' homes, coordinating with interdisciplinary team and referring agencies, administering care, performing initial and ongoing assessments, implementing and revising the plan of care, educating patient/family about disease process and end-of-life care, and coordinating services with IDT. Requires home health/hospice experience, ability to travel locally, and strong documentation and clinical skills for diverse home environments.
Required Qualifications
- Current state RN license
- Graduate of an accredited Nursing program (Diploma, Associate, or Baccalaureate)
- Reliable transportation and auto liability insurance
- Ability to travel locally
- Prolonged sitting/walking with ability to lift up to 50 lbs and move patients
- Proficient communication and interpersonal skills
- Access to Hepatitis consent/declination
