RN Team Leader- Hospice- Night Shift

Provide leadership, mentorship, and daily support to hospice nursing staff while delivering direct patient care as needed. Facilitate interdisciplinary collaboration to support patient and family goals of care, and promote strong communication among team members. Address and resolve patient, family, and staff concerns in a timely and professional manner, and lead performance improvement initiatives to achieve quality outcomes. Coordinate staffing assignments and workflow to ensure seamless care transitions. Conduct staff performance evaluations, including orientation, coaching, and annual reviews, and implement action plans for improvement and compliance. Ensure adherence to HR policies, regulatory standards, and clinical best practices while managing resources responsibly. Represent hospice leadership in internal and external initiatives as directed.
