Job Summary
RN/SW Case Manager provides discharge planning and care coordination across inpatient, observation, and emergency departments; conducts holistic psychosocial assessments; collaborates with multidisciplinary teams to develop timely post-acute care plans, coordinates post-acute services and community resources, advocates for patient and family needs, and documents evaluations, plans, and barriers to progression to ensure timely discharge and continuity of care.
Required Qualifications
- Master's degree
- 2+ care management experience
- 2+ social work experience
- 2+ years in care management or related field (preferred)
- Experience with discharge planning and post-acute care coordination (preferred)
- Knowledge of health care guidelines and community resources (preferred)
