Job Summary
Oversee a caseload of 100-175 home care patients by providing direct supervision to RNs, LPNs, MSWs, and HHAs while ensuring compliance with state and federal regulations. Conduct clinical field supervision, manage performance evaluations, and coordinate multidisciplinary case conferences to document patient care effectively. Monitor staffing for day, evening, weekends, and holidays, including participation in on-call rotation for weekend, night, and holiday coverage. Manage complex patient situations, review clinical records, and assist in formulating policies and continuing education programs. Interview, select, and retain qualified staff while promoting positive customer service internally and externally.
Required Qualifications
- High School diploma or equivalent
- Registered Nurse with active unrestricted license in the State of Nevada and a graduate of an approved school of professional nursing
- Current CPR certification (or obtain within 30 days of hire)
- 2+ years of experience in Home Health patient care setting, providing direct patient care
- 1+ years of performance management with supervisory experience
- Experience working with an electronic medical record application
- Demonstrates thorough knowledge of current nursing practice, professional standards of care, and state & federal regulations regarding home care
- Basic PC skills to include Windows, Outlook or other email system
- High level of critical thinking skills to analyze multiple sources of information to formulate logical conclusions and actions
- Proven ability to plan, organize, prioritize and direct the work of others and communicate objectives clearly
- Proven excellent observation skills, verbal & written communication skills, attention to detail and demonstrated competence in core leadership skills
- Demonstrated ability to successfully utilize clinical information systems for operation and performance management functions
- Valid Nevada driver's license and maintain personal auto insurance coverage
- Must be able to lift 50 lbs
