Job Summary
Conduct comprehensive initial, semiannual, and annual nursing assessments and triage participants in the PACE Clinic. Execute interventions per care plans, coordinate with the interdisciplinary team, and implement state Nursing Home Level of Care summaries. Administer medications, evaluate participant complaints, and communicate findings to physicians and the multidisciplinary team. Provide education to participants and caregivers regarding care plans and treatments. Typical hours are Monday through Friday, 8:00 am to 4:30 pm. Requires a California RN license, CPR certification, and one year of experience with the frail or elderly.
Required Qualifications
- Graduate of a school of professional nursing program
- Current California Registered Nurses License
- CPR certification
- Able to clear fingerprints through DOJ and FBI
- Able to clear drug screen clearance
Desired Qualifications
- Experience in working with the frail or elderly population
- IV certification
