Job Summary
Accurately assesses residents' health status and needs on admission and ongoing, develops and revises the care plan with an interdisciplinary team, documents assessments and interventions (including medication administration and IV fluids), communicates plans to IDT members, residents, and families using SBARR, monitors care plan delivery, addresses issues through QAPI, assists with ADLs, and remains current in nursing practices; may be assigned other duties. Requires nursing education from an accredited program and an active RN license in the working state; CPR/AED certification required or will be obtained within six months; long-term care experience preferred; ability to stand/walk and perform patient transfers with proper technique.
