Job Summary
RN - SNF role in New York focusing on delivering comprehensive nursing care in SNF/hospital settings. Responsibilities include maintaining patient safety, assessing conditions (biophysical, psychosocial, age-specific needs), documenting status per unit procedures, developing and implementing individualized plans of care, collaborating with the health care team, executing the nursing plan of care, evaluating patient outcomes, facilitating patient education and discharge planning, and maintaining Board of Nursing educational requirements. Requires active RN license, primary source verification, AHA BLS, and LTC/Med-Surg experience; IV certification preferred. 40 hours per week, 8-hour days, 5 days per week in New York, NY.
