Job Summary
Provide individualized care for residents including assistance with grooming, bathing, feeding, toileting, and medication administration while maintaining skin integrity and supporting restorative care. Monitor clients for changes in condition or safety risks such as falls, promptly report concerns to licensed personnel, and document updates accurately in the EMR. Collaborate with the interdisciplinary team to support safe care transitions, discharge planning, and regulatory compliance. Work the 7am-3pm shift on alternating weekends with base pay plus differentials.
Required Qualifications
- Active RN or LPN license in the state of Illinois
- Must be able to frequently push, pull, move 20-30 lbs., and occasionally lift a minimum of 50 pounds
- Must be able to read, write, speak, and understand the English language
- Must be able to cope with the mental and emotional stress of the position
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility
- Must stay on unit until relieved by oncoming staff
- Must meet the general health requirements set forth by the policies of this facility

- Ability to communicate effectively both verbally and in written form with a diverse group of people
