Deliver in-home pediatric nursing care to patients in Texas. Provide care within the scope of practice per licensure/certification, complete admission/transfer/re-certification/discharge paperwork, maintain updated clinical records, and evaluate nursing service effectiveness. Communicate with physicians regarding care plan changes and report alterations to the nurse supervisor. Mentor/coaching support to new staff during orientations; educate patients and caregivers on care needs; supervise LPNs and Aides in the home; participate in QAPI; complete mandatory training. Ensure confidentiality and compliant practice; assess patient status and home safety; incorporate multidisciplinary data into nursing assessments; coordinate with family to implement advanced directives if indicated. Participate in committees and perform other duties as assigned. This role requires travel to patients’ homes; shift lengths may vary (8, 10, or 12 hours) and may include any day of the week. Requires an English-proficient communicator, ability to lift up to 35 pounds, and valid driver license with insurance.
