Job Summary
RN Senior Director of Case Management oversees the daily operations of the Care Management department, including Discharge Planning, Care Coordination, and Social Services; partners with physicians, nursing leadership, and executives to optimize patient outcomes, reduce readmissions, and ensure regulatory compliance; leads technology change management initiatives to enhance staff efficiency and patient outcomes; develops and monitors operational budget; mentors interdisciplinary teams and ensures adherence to federal, state, and regulatory requirements; drives seamless care transitions and utilization management in collaboration with Utilization Management.
Required Qualifications
- Registered Nurse (RN) with acute hospital nursing experience required
- Bachelor Degree in Nursing required
- Director of Case Management in an Acute Hospital Setting required
- Master’s Degree preferred
- Certification in Case Management (CCM/ACM) preferred
