Job Summary
Coordinate individualized, evidence-based heart failure care plans, educate patients, and facilitate transitions of care as a Nurse Navigator in the Heart Failure Clinic (PRN). Supports disease management through patient education, follow-up, and care coordination to improve outcomes and reduce readmissions. Partners with providers and interdisciplinary teams to ensure timely, high-quality care; ensures accurate documentation in EPIC; contributes to quality/safety initiatives; and supports program growth and community awareness. Requires BSN and an active RN license; standing and walking throughout shifts; Mercy Hospital South location in St. Louis, MO.
Required Qualifications
- Graduate from an approved school of nursing
- Current RN license in the state of practice
- Bachelor of Science in Nursing (BSN)
- 2–3 years nursing experience
