RN providing total nursing care on an intermittent visiting basis across settings (home care-oriented) within Riverside Health System. Responsible for assessment, planning, implementing, and evaluating patient care, supervising non-professional personnel, coordinating with interdisciplinary teams, and supporting hospice philosophy. Develops initial and ongoing assessments, plans of care, and documentation to ensure timely billing and alignment with patient needs and terminal diagnoses, while addressing age-related needs and cultural considerations. Utilizes communication, critical thinking, problem solving, and decision making to deliver comprehensive care and educate patients and families, including end-of-life care and symptom management, with routine delegation of tasks to LPN/LVN and Home Health Aides; collaborates with physical, speech, and occupational therapy and social services to meet patient goals.
