Job Summary

RN Restorative role responsible for developing, implementing, and evaluating individualized restorative nursing programs based on resident assessments and therapy recommendations. Collaborates with Physical, Occupational, and Speech Therapy to carry over restorative programs after skilled therapy discharge; assesses residents for restorative potential and monitors progress toward functional goals. Trains, supervises, and mentors restorative aides and nursing staff in proper techniques and documentation; ensures restorative programs are carried out safely and consistently. Documents progress and outcomes per facility policy and participates in care planning, interdisciplinary team meetings, and restorative rounds. Identifies changes in resident condition and communicates with nursing leadership and providers; ensures restorative documentation supports MDS accuracy and care plan goals; promotes resident independence, dignity, and person-centered care; assists with survey preparation and CMS restorative care compliance.

Required Qualifications

  • Current, active Registered Nurse (RN) license in good standing in Ohio
  • Minimum of 2 years of nursing experience, preferably in long-term care or skilled nursing
  • Working knowledge of restorative nursing principles and CMS guidelines
  • Strong assessment, communication, and organizational skills
  • Ability to educate and supervise staff effectively
