RN Rehab

Assess patient physical, psychosocial, cultural, and spiritual needs through observation, interviews, and medical record reviews. Develop and implement patient plans of care while evaluating responses to ensure optimal outcomes. Demonstrate competency in therapeutic interventions including IVs, wound care, medication administration, catheterization, and specimen collections. Provide patient and family education based on learning readiness and document ongoing status, interventions, and outcomes accurately. Collaborate with physicians, therapists, and the interdisciplinary team to facilitate discharge planning and advance return to independence. Maintain confidential patient charts and utilize information technology to support decision-making.
