Job Summary
Serve as the primary champion for patients in Womens and Childrens Services within the Case Management pathway, coordinating between hospital and post-acute services to ensure safe, clear hand-offs and secure essential resources. Utilize clinical expertise to develop care plans aligned with treatment options, advocate for efficient use of clinical resources, and guide Medical Records documentation to reflect best possible outcomes. Maintain interdisciplinary collaboration to remove barriers to care and support patient and family needs throughout the continuum of care.
Required Qualifications
- Licensed Registered Nurse credentialed from the Kansas Board of Nursing obtained prior to hire date or job transfer date required.
