RN, Registered Nurse -Transplant Case Manager

Job Summary
Assess members with potential for solid organ or bone marrow transplant, end stage renal disease, and hematologic malignancies. Apply the nursing process to actively case manage transplant members from referral through post-transplant closure. Utilize critical thinking to problem-solve and make knowledgeable recommendations for needed actions. Document all activities in the appropriate database and maintain collaborative relationships with health care teams. Serve as a member and provider advocate by educating clients and guiding them through the transplant process. Provide benefit and health information to enable informed decisions and act as a clinical resource for all Quantum Health work groups. Maintain contact with clinical staff for case transfers and provide updates to internal client executives.
Required Qualifications
- Active, unrestricted, and unencumbered professional license in good standing with the state board of licensure in one of the applicable clinical disciplines: Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Pharmacist (PharmD), or other licensed healthcare professional authorized to conduct an assessment independently
- Multi-State License (MSL) issued through the relevant licensure compact (e.g., Nurse Licensure Compact for RNs) where applicable
- Willingness and ability to obtain and maintain additional state licenses as required to support members served by the organization within 90 days of hire
- Maintenance of required licensure, including timely renewal and compliance with state board regulations
- Minimum of two years clinical experience with direct patient care
- Outstanding computer skills including Microsoft applications
- Excellent critical thinking skills
- Excellent verbal and written communication skills
- Excellent time management skills
- Demonstrated ability to work autonomously
- Commitment to work within company ethics and policies to protect and take care of company and member's data
- Legally authorized to work in the United States on a permanent and ongoing future basis without requiring sponsorship
Desired Qualifications
- Certified Case Manager (CCM) credential from the Commission for Case Manager Certification
- Bachelor's of Nursing
- Solid organ or bone marrow transplant experience
