Job Summary
Utilizes the nursing process to identify and achieve patient goals including assessment, planning, implementation, and evaluation. Assesses physical, psychosocial, and environmental factors affecting a patient’s health to develop comprehensive nursing care plans that attain desired health outcomes. Collaborates with the interdisciplinary team to assure personal care, medical care, and rehabilitation for optimal patient health and well-being. Teaches patient/caregiver aspects of health care and disease management appropriate to their needs and understanding.
Required Qualifications
- Unencumbered current Registered Nurse license in the state where the HomeCare agency is located or compact state, if applicable.
- Minimum one (1) year experience as a RN / professional nurse.
Desired Qualifications
- Experience in home care is desirable.
