Job Summary
Conduct comprehensive assessments of patients and families to determine home care needs, including physical exams and history taking. Initiate and revise plans of care with physicians, administering medications and therapeutic interventions as prescribed. Perform high-tech nursing procedures such as catheter care, ostomy management, and negative pressure wound therapy while adhering to infection control protocols. Counsel patients and families on health instructions and discharge planning needs. Supervise home health aides and LPNs/LVNs, ensuring care quality every 14 and 30 days respectively. Coordinate care with community providers and specialists, maintaining clinical documentation and communicating status changes to the medical team. Participate in on-call duties and ensure availability of necessary equipment and supplies.
Required Qualifications
- Bachelor's degree of Nursing from an accredited college/university
- Current active and unrestricted nursing license
- Valid driver's license
- Current certification in Basic Life Support for Health Care Professionals
- Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist
- Basic knowledge of computer operating systems and software applications to communicate and document care
- Prior experience working with the Adults/ Geriatric population
- Prior experience working with the Special Needs population
- Pediatric experience
- Experience in pediatric nursing from birth through late teens
- emphasis on community health education/experience

- Pediatric experience is a plus
