Job Summary
Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs, including physical assessments, history taking, and nursing diagnoses. Develop care plans establishing goals based on these assessments, regularly re-evaluate status, and initiate or revise plans as patient needs change. Document pain and symptom management outcomes, communicate with physicians regarding conditions, and teach self-care techniques to patients and caregivers. Collaborate with the interdisciplinary group and travel throughout the Bristol Hospice Service Area. Requires one year of acute care experience, current licensure, and flexibility in work hours.
Required Qualifications

- at least one (1) year of acute care experience in an institutional setting
- current licensure in the state
- Must understand the philosophy of hospice concept and understand needs of the terminally ill
- Must be flexible in work hours
- Must demonstrate nursing skills per competency checklist
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
- Ability to travel throughout the Bristol Hospice Service Area
Desired Qualifications
- One (1) year of experience in home care preferred
