Job Summary
Conduct comprehensive assessments of patients and families to determine hospice needs, including physical history and status evaluation. Develop care plans based on nursing diagnoses, establish goals, and include patients and caregivers in the planning process. Initiate and revise plans of care as patient status changes, while documenting pain management and communicating with physicians regarding orders and condition changes. Teach self-care techniques and collaborate with the interdisciplinary group. Requires at least one year of acute care experience, current licensure, and ability to travel throughout the service area. Flexible work hours available with tuition reimbursement, PTO, and medical/dental benefits.
Required Qualifications
- Registered Nurse (RN)
- at least one (1) year of acute care experience in an institutional setting
- current licensure in the state
- Must understand the philosophy of hospice concept and understand needs of the terminally ill
- Must be flexible in work hours
- Must demonstrate nursing skills per competency checklist
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
- Ability to travel throughout the Bristol Hospice Service Area
Desired Qualifications
- One (1) year of experience in home care preferred
