RN / REGISTERED NURSE - MED/SURG TELEMETRY

Facilitate, implement, and evaluate patient Plans of Care while providing timely nursing interventions based on clinical priorities. Assess patient condition changes, communicate relevant information to the interdisciplinary team, and act as a patient advocate for safety and preferences. Collaborate with physicians and staff to promote positive outcomes, review provider orders accurately, and ensure continuity of care throughout the shift. Identify safety risk factors, initiate interventions for fall prevention and skin integrity, and complete required Safety Tracking Tools. Document assessments, interventions, and education provided in the medical record while appropriately delegating to assigned staff. Prioritize responsibilities based on urgency and function independently while seeking resources as needed.
